Saltiest Membership $184

Only fill this out if you are starting a membership! This should only be done, in-person, at "The Float Place".

I would like to enroll in the "Saltiest" Membership program at The Float Place. My membership is $184 per month, and I am to receive four (4) 90 Minute Float Sessions per month. Additional floats are $44 per session. Floats are valid for as long as the membership is active.

The "Saltiest" gets you:
-(4) 90-minute Float sessions per month
-Unused float sessions roll over
-Float sessions are shareable
-Additional credits are $44
-10% off retail + additional services
-Purchaser agrees to a 3-month minimum, card billed until canceled
-To cancel your membership, please email us at floatmembership@gmail.com and you will receive a return email within 24 hours verifying the cancellation
There is a $35 fee for any cancellations or reschedules within 24 hours.

There is a 3-billing cycle minimum. Credit/Debit Cards are kept on file and are charged each month on the same date. This charge may appear as "LI Float LLC." on your statement. There is no cost to cancel the membership, however if you cancel prior to the 3-month minimum you will be charged full retail price for all float sessions used prior. To cancel your membership, please email us at floatmembership@gmail.com and you will receive a return email within 24 hours verifying the cancellation.

I understand there is a 3 month minimum and my card will be charged monthly until I cancel it by emailing Floatmembership@gmail.com Non-payment of 3 month membership minimum requirement will will result in floats (or other services) held in the account being frozen until payment is brought current.*

Any floats that were part of a promotion or special are subject to expiration dates and certain limitations. Promotional floats expire in 42 days and are not shareable. Limit of one (1) promotional float redemption per day. Only two (2) promotional floats can be booked at one time. There is a $35 fee for any cancellations within 24 hours.*

I understand there are no refunds for monthly charges that have processed. *
