Hot and Cold Membership

Only fill this out if you are starting a membership! This should only be done, in-person, at "The Float Place".

I would like to enroll in the "Hot and Cold" Membership at The Float Place. My membership is $59 per month, and I am to receive one (1) 90 Minute Fire and Ice Session per month. Sessions are valid for as long as the membership is active.

The "Hot and Cold" gets you:
-(1) Fire & Ice session per month
-Unused sessions roll over
-Sessions are shareable
-Additional Guest is only $25
-10% off retail + services
-Purchaser agrees to a 3-month minimum, card billed until canceled
-To cancel your membership, please email us at floatmembership@gmail.com and you will receive a return email within 24 hours verifying the cancellation
There is a $35 fee for any cancellations or reschedules within 24 hours.

There is a 3-month billing cycle minimum. Credit/Debit Cards are kept on file and are charged each month on the same date. This charge may appear as "LI Float LLC." on your statement. There is no cost to cancel the membership, however if you cancel prior to the 3-month minimum you will be charged full retail price for all sessions used prior. To cancel your membership, email us at floatmembership@gmail.com and you will receive a return email within 24 hours verifying the cancellation.
