Fire & Ice ($49)

Only fill this out if you are starting a membership! This should only be done, in-person, at "The Float Place".

I would like to enroll in the "Fire & Ice" Membership program at “The Float Place”. My membership is $49 per month, and I am to receive one (1) 90 Minute Fire and Ice Session per month. Additional sessions are $49 each. Sessions are valid for as long as the membership is active. They are nontransferable.

The "Hot and Cold" gets you:
-(1) 90-minute Fire and Ice Session per month
-Unlimited additional 90-minute fire and ice sessions for yourself at $49 each
-In-house monthly perks (10% off retail, membership promotions, events, giveaways, etc.)
-3 month Minimum. Cancel any time after
-Monthly sessions roll over
-One free Guest every session

There is a 3 Month billing cycle minimum. Credit/Debit Cards are kept on file and are charged each month on the same date. This charge may appear as "LI Float LLC." on your statement. There is no cost to cancel the membership, however if you cancel prior to the 3-month minimum you will be charged full retail price for all sessions used prior. To cancel your membership, please email us at floatmembership@gmail.com and you will receive a return email within 24 hours verifying the cancellation.

I understand there are no refunds for monthly charges that have processed. *

I understand that I must be present for the Fire & Ice Sessions in order for a guest to come for free. My credits are nonshareable.*
