PLAYHOUSE PARADISE LLC CAMP PARTICIPATION AGREEMENT

We are excited to have you and your child join us for a memorable and enriching experience. Before we get started, please take a moment to review the following important information. By signing this agreement, you acknowledge the terms and conditions outlined below.
IMPORTANT NOTICE:This release is valid for one full year from the date it is signed. After that period, we will require an updated form. Please note that one form is required per family.
1. Acknowledgement of Participation and Legal AgreementBy signing this document, you confirm that you are at least 18 years old and the parent or legal guardian of the child or children listed on this form. You are entering into a legal agreement with Playhouse Paradise LLC, and by doing so, you are waiving certain legal rights, as described in this agreement.
2. Understanding of RisksWhile we aim to create a fun and engaging environment, we want you to be aware of the inherent risks associated with participation in camp activities. These include, but are not limited to, minor injuries such as cuts, bruises, sprains, and other accidents. By signing this agreement, you acknowledge and accept the potential risks involved in participating in camp activities.
3. Health and SafetyPlayhouse Paradise LLC is committed to maintaining a safe environment for all participants. However, we cannot be held responsible for illnesses, accidents, or injuries that may occur. If your child is feeling unwell or exhibits signs of illness, we ask that you keep them at home. Adherence to all health and safety protocols is required.
4. Camp Rules and ExpectationsAll participants (children and adults) are expected to follow the rules and guidelines established by Playhouse Paradise LLC. Failure to do so may result in removal from camp activities. We reserve the right to dismiss any participant whose behavior poses a safety risk or disrupts the camp experience for others.
5. Permission for Use of Photos and MediaBy signing this agreement, you grant Playhouse Paradise LLC permission to photograph and video record your child during camp activities. These images may be used in promotional materials, including but not limited to websites, social media, and brochures. If you do not wish to have your child’s image used, please inform us in writing.
6. Medical and Health InformationFor the safety of all participants, we require a completed Health Appraisal Form by May 31, 2025. This form will include emergency contact information and any relevant medical history. Additionally, if your child requires medication during camp hours, you must provide written authorization and instructions.
7. Pick-Up and Drop-Off PoliciesAuthorized adults listed on your registration form will be permitted to pick up your child at the end of each camp session. Please inform us in advance if you need to update the list of authorized individuals. If someone other than the designated adult is picking up your child, we require prior written notice.
8. Liability WaiverYou acknowledge and accept that, by participating in camp activities, Playhouse Paradise LLC, its employees, agents, and volunteers cannot be held liable for any personal injury, property damage, or loss of personal items unless caused by gross negligence or intentional misconduct on our part.
9. Cancellation and Emergency ProceduresIn the event of a camp cancellation due to unforeseen circumstances, such as extreme weather or public health concerns, Playhouse Paradise LLC will make reasonable efforts to reschedule or provide a credit for future sessions. If your child is unable to attend due to illness or other personal reasons, you must notify us as soon as possible.
10. Behavioral ExpectationsWe strive to maintain a positive and respectful environment for all participants. We expect children to treat others with kindness and respect. If a child’s behavior is disruptive or unsafe, we reserve the right to remove them from camp activities. In such cases, no refund will be issued.
11. Limitation of LiabilityIn the event of any dispute or claim arising from participation in Playhouse Paradise LLC camp activities, the maximum liability of Playhouse Paradise LLC will be limited to the amount paid for the camp session or $1,000, whichever is less. We are not liable for any indirect, incidental, or consequential damages.
12. Force MajeureIn the event that Playhouse Paradise LLC is unable to perform its obligations due to circumstances beyond our control (such as natural disasters, pandemics, or government restrictions), we will work with you to reschedule or offer credits, but we will not be held liable for any losses.
13. Dispute ResolutionAny disputes arising from this agreement or participation in camp activities will be resolved through binding arbitration, rather than through a court trial. This process will be held in accordance with the rules of the American Arbitration Association.
