You have an opportunity to participate in a Pathfinder Program. Safety is our first concern and every activity is monitored to maintain the high standards we commit to in our accreditation. To be eligible to take part, you must sign this release form.
In general terms I understand:
1) that there is potential risk in any physical activity;
2) that I choose to participate;
3) that I agree to follow all rules and safety precautions;
4) that I am healthy enough for a moderate level of physical activity;
5) that I will alert the facilitators of any special needs or concerns I might have; and
6) that I consent to any necessary emergency medical treatment (coverage is the responsibility of the participant).
I have and do hereby Release and Forever Discharge Pathfinder Inc., its employees and directors (herein collectively “Pathfinder Inc.”) from any and all liability, actions, causes of action, claims, and demands of any nature whatsoever which we now or may have as a result of my participation in this program. I agree to indemnify and hold harmless Pathfinder Inc. from any and all damage, loss, or liability occurring by reason of any injury to me or my property caused by acts of omission, neglect, or wrong doing of Pathfinder, Inc. or myself, which may arise out of my participation in this program. I hereby assume all risk of injury associated with this program.
