REQUIRED FOR EACH CHILD
I understand that the programs and activities offered at and by Controller may involve exposure to materials, videos, and language deemed inappropriate to some children. Controller will monitor, and regulate the activities of its patrons, and will try and minimize exposure to materials, videos, and language to children. However, with the nature of gaming, open-play format, and competition, there is the chance that inappropriate images, language, and content may be viewed and/or heard by a minor.
I assume the risks and hazards incident to such participation. I understand that I am responsible for determining and disclosing whether I and/or my minor child should be limited to certain game titles, activities, and/or foods.
I authorize my child to play the following titles as organized The Entertainment Software Rating Board (ESRB; http://www.esrb.org/). Controller will limit the games available to play based on the parental limits checked below.

Content is generally suitable for all ages. May contain minimal cartoon, fantasy or mild violence and/or infrequent use of mild language.
TEEN
Content is generally suitable for ages 13 and up. May contain violence, suggestive themes, crude humor, minimal blood, simulated gambling and/or infrequent use of strong language.
EVERYONE 10+
Content is generally suitable for ages 10 and up. May contain more cartoon, fantasy or mild violence, mild language and/or minimal suggestive themes.
MATURE
Content is generally suitable for ages 17 and up. May contain intense violence, blood and gore, sexual content and/or strong language
