The undersigned adult, named below agree to the conditions outlined below on behalf of the minor participant.
There are risks and dangers associated with participation in martial arts events and activities which could result in bodily injury or death. Initial Here
I understand and agree that, if at any time, the minor feels anything to be UNSAFE; they will immediately take all precautions to avoid the unsafe area and REFUSE TO PARTICIPATE further. Initial Here
If the minor has a disability, illness, pregnancy, or is currently seeing a therapist, I promise to consult with their physician or therapist before they participate in martial arts training. Initial Here
For safety, the minor will practice in a considerate and conscientious manner and strictly follow all rules of the dojo. Should they break any of these rules, I understand that it is the decision of the head instructor whether they may continue training. I will abide by this decision. Initial Here
I further acknowledge that training may be recorded by video or by other means and I agree to the minor being recorded in whatever format is being used and that Aikido West may use images in any format for all legal purposes both commercially and non-commercially. Initial Here
I also understand that the minor may be exposed to individuals in the dojo that may have an infectious disease. I acknowledge that I have read the documents COVID-19 Rules for Indoor Training and "Aikido West Blood Borne Pathogen Policy" and agree to have the minor abide by these rules. Initial Here
I/We release, waive, discharge, and covenant and agree not to sue or make claim against Aikido West or any of its affiliated organizations, their respective agents, officers, directors, instructors including but not necessarily limited to visiting, guest and substitutes, employees, volunteers, sponsors, members, other participants, their parents, guardian(s), supervisors, and if applicable, owners, lessors, and lessees of any premises used by Aikido West or any of its affiliated organizations, all of whom are hereinafter collectively referred to as “Releasee”, from any and all claims, demands, losses, or damages on account of injury, including permanent disability and death or damage to property, caused or alleged to have been caused in whole or in part by the negligence of the Releasee or otherwise. Parent(s) or legal guardian(s) of minor participant (age 17 and below) additionally agree that they have instructed and advised the minor participant as to the above warnings and conditions and their ramifications, and that they consent to the minor’s participation.
