Facility Access Agreement*

This Play Facility Access User Agreement (“Agreement”) is made and entered into on the date of execution below (“Effective Date”) between user and Sensory Innovations LLC (“Sensory Innovations”) to access Play Facility (“Facility”) and services offered by Sensory Innovations.

Facility Access*

Your access to and use of the Facility (and resources contained therein) is conditional upon your acceptance and compliance with the terms, conditions, notices, policies and disclaimers contained in this document and other documents received by Sensory Innovations (collectively as “Terms of Use”). Your use of, and/or access to, the Facility constitutes your agreement to the Agreement and Terms of Use. If you do not agree to the Agreement and Terms of Use, please do not utilize the Facility.

You hereby agree to relinquish all claims, suits, attorney fees, damages, liability and any and all future rights to the same based on any and all negligent acts of Sensory Innovations and any and all of the named agents and employees entirely harmless and free of liability for any and all negligent acts on the date of signature and all future visits to the Sensory Innovations facility up to a 1 year period from the signature date. You have the opportunity, if you wish, to speak with a representative of Sensory Innovations for further explanation of the terms contained herein before signing.

You hereby acknowledge and agree that I have carefully read and fully understand the terms of this Agreement, and that I have been afforded an opportunity to request further explanation of the terms of this Agreement.

Cancelations (Host information)

Facility reservations may be cancelled at any time for any reason. Sensory Innovations does not offer refunds, the event may be moved to a different day with a 48 hour notice, or funds may be used towards a Sensory Innovations membership or visit options.

Photo's (Host information)

I agree as an adult participant, or the Parent/Legal Guardian of a minor participant, that in consideration of being permitted to participate at SENSORY INNOVATIONS LLC, I grant SENSORY INNOVATIONS LLC and its agents, owners, shareholders, directors, partners, employees, volunteers, manufacturers, participants, lessors, affiliates, its subsidiaries, related and affiliated entities, successors and assigns (the “RELEASED PARTIES”) the right and permission to photograph and/or record me or my child in connection with SENSORY INNOVATIONS LLC. I further grant SENSORY INNOVATIONS LLC and all RELEASED PARTIES the right and permission to use the photograph and/or recording for all purposes, including advertising and promotional purposes, in any manner and in any and all media now or hereafter known, in perpetuity throughout the world, without restriction as to alteration. I waive any right to inspect or approve the use of the Photograph and/or Recording. I acknowledge and agree that the rights granted to this release are without compensation of any kind. All Photographs and/or Recordings are exclusive to SENSORY INNOVATIONS LLC. If the participant is a minor, I agree that this Photo Release Form (“RELEASE”) is made on behalf of that minor participant and that all of the releases, waivers, and promises herein are binding on that minor participant. I represent that I have full authority as Parent or Legal Guardian of the minor participant to bind the minor participant to this agreement. I agree that this form will remain in effect until revoked. I understand that it is my responsibility to update this form if I no longer wish to authorize the above uses.

Additional Fees ( Host and All Participants)

Additional Fees may apply if items in OT store or Café get damaged or go missing. This will be charged to the host of the event to the card we have on file.

Additional Charges (Host information)

Our base Special Events package includes up to 20 people (kids and adults) we have a blanket charge of $50.00 that will be charged to the card on file in the case your event goes over 20 people. If time exceeds the base time of 2 hours we will charge the card on file $25.00 for every 30 minutes the party goes over

Clean up (Host information)

Cleanup starts 30 minutes prior to event ending. This is for the main party space. At this time the host is able to collect items they would like to keep. During this time guest are welcome to explore sensory rooms while the main party space is being cleaned.
