Volunteer Waiver Release

 In consideration of being allowed to participate in any way in Seattle Adaptive Sports (SAS) programs, related events and activities, I and/or the minor participant, for myself, and on behalf of my heirs, successors, assigns, personal representatives and next of kin, the undersigned: 

Agree to indemnify and hold SAS harmless, and release SAS from any and all liability for any injury which may be suffered by the below named individual in an SAS activity or event arising in any way connected with participation in SAS events except as may arise out of the sole willful act or sole negligence of SAS, its officers or agents.

 I give consent to SAS to perform a Washington State Patrol background check pursuant to WA RCW 43.43.830 and understand that I can request a copy of my results.

Volunteer Code of Conduct  

 

The purpose of this Volunteer Code of Conduct is to provide both volunteers and participants with clear guidelines to ensure a fun and safe environment for everyone involved. While appropriate actions are often intuitive, it can sometimes be harder than it sounds to be likable and friendly while maintaining necessary boundaries.

 

 As a volunteer you are expected to adhere to the following guidelines:

  • Volunteers will not swear, use derogatory language, or discuss inappropriate topics around or with participants.
  •  All conduct with participants should place the safety and health of the participant first.
  • Volunteers will not sit in or use participants’ wheelchairs or equipment.
  •  Volunteers will not be alone with participants.
  •  Volunteers should at all times act as a positive role model for participants.
  • No alcohol, tobacco or other drug use is permitted at Seattle Adaptive Sports functions.
  • Please refrain from hugging or any physical contact that could be misconstrued as inappropriate. High fives and fist pounds are a great way to greet and congratulate participants.
  • You may be asked to leave if your behavior or actions are not deemed to be in the best interest of Seattle Adaptive Sports programs or athletes. Such decision shall be at the sole discretion of the event leaders.

Media/Photo Release

I hereby authorize and give my full consent to Seattle Adaptive Sports to copyright and/or publish any and all photographs, videotapes and/or film in which I appear while attending SAS practices, events and/or activities. I further agree that SAS may transfer, use or cause to be used, these photographs, videotapes, or films for any exhibitions, public displays, publications, commercials, art and advertising purposes, and television programs without limitations or reservations.

FOR PARTICIPANTS UNDER THE AGE OF 18 

 

This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her/their release as provided above of the Releasees, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless SAS from any and all liabilities incident to my minor child’s involvement or participation in these programs as provided above, even if arising from their negligence.

 

I HAVE CAREFULLY READ THIS AGREEMENT AND UNDERSTAND ITS CONTENTS.  I AM AWARE THAT I AM RELEASING LEGAL RIGHTS THAT OTHERWISE MAY EXIST. BY SIGNING BELOW, I HEREBY REPRESENT THAT I AM AT LEAST 18 YEARS OF AGE AND FULLY COMPETENT TO SIGN THIS AGREEMENT ON MY OWN BEHALF.
