Camp Self-Release Form

Parents and/or Guardians are required to sign in and sign out registrants during our Summer Camps. If you are unable to sign in and/or sign out your child, or give them permission to arrive or leave camp on their own, please fill in the below form to allow your child to sign themselves in and/or out during camp. Please note that the age of self-release is at least 11 years old.
