Optimal K9 Board And Train Accident And Release Of Liability Form

I am aware of the risks connected with the training on this date. I agree to not hold accountable or bring legal action against the company, their officers, agents, or employees. I will inform Optimal K9 of all medical conditions and physicals conditions that pertain to the dog before I release the dog into Optimal K9's Care. This waiver releases the company from all liability relating to injuries or death that may occur on location before, during, or after activity.
 

If the medical or physical condition of the dog disrupts the board and train the handler is required to come get the dog back into their care. (i.e. Not eating, Cherry Eye, Vomiting, persistent diarrhea, period bleeding)

 Please advise that food will be offered every day to your dog during training. If your dog refuses, they will not eat until the next training session. Failure to eat after 3 consecutive days will result in the handlers being notified & the possibility of the dog being returned home. If your dog is unable to continue boarding due to a violation of Optimal K9 boarding procedures. Your dog will be recommended in-home sessions, to ensure that the goals of your dog are still met.

You acknowledge that dogs are unpredictable animals with their own will and drives, and therefore, regardless of training, handling, or environmental circumstance, Optimal K9 cannot protect against every potential injury, risk, infection, parasite or accident to your furry companion, to other persons participating in the training or on the property where the training is taking place. These may include dog attacks, dog bites and the responsive actions and complications that arise from them, and you understand and acknowledge that such risks are inherent and cannot be eliminated.
I acknowledge the risk involved with this activity including, but not limited to, muscle tears, sprains, and other physical injuries. I certify that my participation is voluntary, and I have been made aware of the risks. Additionally, I do not have any conditions that may increase the likelihood of injury on my animal.

 

Please advise some dogs do not kennel well! This means they do not operate at 100% when away from their normal routine & human counterparts. That being said behaviors like obsessive paw licking, self-harm, loss of appetite, weight loss destruction of crate etc.…. Are all possibilities when picking boarding as a program for your furry companion. If your dog is abnormally anxious or barely enjoys eating food in the comfort of your home Optimal K9’s Board and Train may not be right for you. Optimal k9 trainers will do everything possible to communicate & prevent any mishaps or injuries that may occur while your dog is in company care please understand that all risks cannot be eliminated and optimal k9 will not be held responsible.

 
I will make every effort to obey the company personnel, all safety rules, and will ask for clarification if needed.

 

This Agreement is intended to shift the risk of participating in dog training to you. Do not sign it unless you are willing to assume the risk to yourself, your dog, and others accompanying you of your participation in dog training, including the associated risks of bodily injury, death or property damage while participating in the dog training and related activities on the property of Optimal K9 , in your own home, or in a public space.
