STATEMENT OF LIABILITY/LIABILITY WAIVER

The undersigned hereby acknowledges that I am a student at John Abbott College and wish to participate in a trip/activity to:

Place:  Winnipeg, Manitoba

Date:    April 24 - 27, 2025      

and understand that I am not required by the College academic program to make this trip, but that this extra-curriculum activity (trip) is organized voluntarily by individual staff of the College Student Activities Department, as a para-service for which the College is not legally responsible.

In consequence whereof, I hereby agree to hold harmless, individually and severally,

and John Abbott College and/or its employees, agents, préposés, mandatories and/or their heirs or successors and legal representative and/or the liability insurers of the aforementioned parties, for any and all liability for damages of any kind or nature whatsoever suffered due to my participation in this trip, it being expressly understood and agreed that I accept all risks involved.  I understand that the aforesaid trip, even under the safest conditions possible, may be hazardous.

Moreover, it is understood that I agree that the above people involved in this trip shall not be considered to be a préposé or an employee of John Abbott College acting in the scope of his (their) function(s) for the organization and/or execution of this trip and being in no way under the control or supervision of the College while doing so.

My participation in this activity and event is made on my own will, at my risks and perils, after the nature of same has been duly explained prior to my engagement in it.

Please respond to the following questions. If you do not agree to any of the conditions, you will not be able to travel on this trip or participate in this competition.



I agree that I (my son/daughter) must respect the student code of conduct of John Abbott College. Any breach may result in disciplinary actions up to and including dismissal the trip and prompt return to Montreal at my (their) sole expense.



I understand that this trip is partially subsidized by student funds and agree that if I (my son/daughter) cancel, I (they) must refund the subsidized portion of the fees and or flight costs and travel insurance paid on my (their) behalf.



I understand that I (my son/daughter) must have the necessary documents required to Winnipeg and return back to Montreal. Since all individuals may have unique circumstances, it is your responsibility to verify what is required.



I understand that I (my son/daughter) must be covered by travel insurance to Winnipeg. This insurance must have the following components: Trip Cancellation, Trip Interruption, Baggage and Emergency Medical coverage. If covered by travel insurance, I (they) will provide proof to the College. If not covered by travel insurance, I understand that I (they) must purchase travel insurance on my (their) own.



I hereby authorize Canada Ethics Cup National Championships and/or John Abbott College and its representatives to photograph me (my son / daughter) and/or produce audio recordings of my (my son/daughter) voice and/or video recordings of me (my son/daughter) and/or use my (my son/daughter) name in relation to postings, social media posting and for purposes of print media.
