RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT
Dear Parents/Guardians:
The Full Day Summer camp will be BIKING to Woodlands Water Play Park (St. Albert Splash Park), located at 165 Sturgeon Rd, St. Albert, AB T8N 3G1 on August 7th, Weather permitting. If weather is bad we will walk to Servus Place instead.
The Full Day Summer camp will also be going to Millennium Place, 2000 Premier Way, Sherwood Park, AB T8H 2G4 on August 9th/2024 @ 10:30 am.
Be sure your child is prepared for the weather and brings proper footwear, and snacks. They will be going rain or shine so hats, sunscreen, and/or optional mosquito repellent is recommended. To give permission for your child to participate and travel to the destination, please fill out the form below.
This is a binding legal agreement. Clarify any questions or concerns before signing. As a participant in the sport of gymnastics and the spectating, orientation, instruction, activities, competitions, programs, travel, and services of Dynamyx Gymnastics Club (collectively the “Activities”), the undersigned acknowledges and agrees to the terms outlined in this document.
Dynamyx Gymnastics Club, Alberta Gymnastics Federation and their respective Directors, Officers, committee members, members, employees, coaches, volunteers, officials, participants, agents, sponsors, owners/operators of the facilities in which the Activities take place, and representatives (collectively the “Organization”) are not responsible for any injury, personal injury, damage, property damage, expense, loss of income or loss of any kind suffered by a Participant during, or as a result of, the Activities, caused in any manner whatsoever including, but not limited to, the negligence of the Organization.
