I, despite all reasonable precautions implemented for safety, am fully aware of and appreciate the risks, including the risk of catastrophic injury, paralysis and even death, as well as other damages and losses associated with participation in the programs or activities. I knowingly and willingly assume all such risks. Consequently, I hereby for myself, heirs, executors and administrators, do waive and release any and all rights and claims for damages against the owners, operators, coaches and other members of MD Ninja Concord LLC from personal injury or accident of any sort or nature suffered by me, the undersigned, by reason of participation or membership in classes, lessons or any programs or activities of MD Ninja Concord LLC.

In, addition, I, the below named minor's parent and/or legal guardian, understand the nature of these activities and the minor's experience and capabilities and believe the minor to be qualified, in good health, and in proper physical condition to participate in such activity. I hereby release, discharge, covenant not to sue, and agree to indemnify and save and hold harmless each of the releasee's from all liability claims, demands, losses, or damages on the minor's account, including negligent rescue operations. I further agree that if, despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the releasees named below, I will indemnify, save, and hold harmless each of the releasees from any litigation expenses, attorney fees, loss liability, damage, or cost any may incur as the result of any such claim.

COVID-19 Parent Agreement

I, acknowledge that I, my family members and our minors who are participating in activities at USA Ninja Challenge will self-screen before coming to USA Ninja Challenge for any class, event or activities.  We will not participate if our minor or a family member exhibits any of the following symptoms:  cough, sore throat, difficulty breathing, loss of taste or smell, chills, repeated shaking with chills, muscle pain, headache, diarrhea, fever and/or a measured temperature equal to or greater than 100.0 degrees F.

I understand that:

  • Face coverings are not required in the facility, however some staff members may choose to wear them while engaged in activities in the lobby or offices and, possibly, while instructing students.  If I want my child to wear a face mask, USA Ninja Challenge supports and allows that.  
  • Every student should bring their own filled water bottle to class..
I further acknowledge, understand, appreciate and agree that my and/or my child's participation may result in possible exposure to and illness from infectious diseases, including, but not limited to, MRSA, Influenza, and COVID-19.  While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist.  I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my and my child's participation and exposure.

Subject: Competitions, workouts, team activities and functions at any USA Ninja Challenge facility or the location of any USANC competition or activity. I grant to MD Ninja Concord LLC, its representatives and employees the right to take photographs of my child in connection with the above-identified subject. I authorize USA Ninja Challenge, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that USA Ninja Challenge may use such photographs of my child with or without his/her name for any lawful purpose, including publicity, illustration, advertising, and web content. I have read and understand the above.

USA Ninja Challenge has established a dress code for participants to ensure the safety of our students, party guests and coaches. Student and party participants should be dressed appropriately for the activity in which they participate. Dress codes will be enforced. All students and guests should wear: – Short sleeve T-shirt or tank and shorts – Tight-fitting capri length pants are acceptable – Bare Feet – no socks Long hair must be put up with hair ties or scrunchies. Do not use bobby pins, as they have been known to cause injuries to the head, if fallen upon. Never wear the following: – Necklaces, dangling earrings, bracelets, belts, watches, fit bits, etc. All of these items get caught on clothing, equipment or other people and will break or cause injury. – Long sleeves – can get caught or hinder a coach from spotting – Pants – can get caught or pulled down easily – Hoodies, sweat shirts, dresses or other loose clothing. All of these can cause an injury to a coach when spotting or to a student if clothing gets caught on equipment. If you have a question about whether or not a particular piece of apparel is acceptable, please ask before sending your child to a ninja party.
