Release and Waiver of Liability
BLUE ZOO SPOKANE

By signing this release and waiver of liability (the “Release”), you do so in favor of BlueZoo, Inc., its directors, officers, administrators, employees, and agents (collectively referred to herein as “BlueZoo”) and give up the right to sue BlueZoo for any injury or damages, of whatever kind or nature, that may arise from or relate to your participation in the programs and activities offered by BlueZoo (the “BlueZoo Activities”).

I, [FIRST NAME] [LAST NAME], hereby sign this Release on behalf of myself, my personal representatives, heirs, next of kin and anyone who obtains rights from me, and on behalf of my child(ren).

I desire to participate in the BlueZoo Activities and understand that such participation may involve risk of bodily harm, serious injury, death, or personal property damage or loss. For this reason, I do hereby freely, voluntarily, and without duress execute this Release under the following terms:

1. Waiver and Release. I do hereby release and forever discharge and hold harmless BlueZoo and its successors and assigns from any and all fault, negligence, liabilities, costs, claims, actions, demands, penalties, losses, lawsuits, arbitrations, or damages of whatever kind or nature, either in law or in equity, which arise or may hereafter arise, in any way, from my participation in the BlueZoo Activities.
   
   
2. Medical Treatment. I do hereby release and forever discharge BlueZoo from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with my participation in the BlueZoo Activities.
   
   
3. Assumption of Risk. I do hereby expressly and specifically acknowledge and assume the risks inherent in the BlueZoo Activities and release BlueZoo from all liability for harm, injury, illness, death, or damage to personal property that arises from or relates in any way to my participation in the BlueZoo Activities.
   
   
4. Photographic Release. I do hereby consent to the use of my likeness in print, photographic, digital, electronic, or social media by BlueZoo in connection with BlueZoo’s marketing, advertising, and promotion of the BlueZoo Activities and the BlueZoo brand. I give BlueZoo permission to use my personal information for marketing and promotional purposes, including, but not limited, to newsletters, emails text offers, discounts and promotions.
   
   
5. Scope. I expressly agree that this Release is intended to be as broad and inclusive as permitted by the laws of State Washington and that this Release shall be governed by and interpreted in accordance with the laws of the State Washington that in the event any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release, which shall continue to be enforceable.
6. Allergy warning: the food fed to the animals may contain shellfish, seafood, peanuts and other food allergies. Animals, their dander, feathers, saliva, animal feed or bedding may cause allergic reactions in some people.

I have read and fully understand this Release and that I am assuming all risks inherent in participation in the BlueZoo Activities. I voluntarily sign my name, and thereby state my acceptance of the above provisions.
