INFORMED CONSENT/PERMISSION FORM FOR EDUCATION TRIPS
(Students Under 18 Years)
is arranging a visit to Blue Mountain Resort on . Students (under the supervision of teachers/group leaders and parent volunteers) will have the opportunity to participate in one or more of the following: Plunge aquatic; snowshoeing; tubing; skating; ride the alpine coaster; canopy climb net adventure; team challenges or instructional type activities.
THIS FORM MUST BE READ AND SIGNED BY A PARENT OR LEGAL GUARDIAN OF A PARTICIPATING STUDENT.
ELEMENTS OF RISK:
Educational activity programs involve certain elements of risk. Injuries may occur while participating in these activities. The following list includes, but is not limited to, examples of the types of risk which may result from participating in the visit to Blue Mountain Resort:
variation or changes in the terrain both inside and out; unpredictable or variable weather conditions; exposure to the elements: slipping, tripping, falling, including falling from heights or through thin or cracked ice; becoming lost or separated from the group; aggravation of pre-existing injuries, conditions or congenital anomalies; changes or variations in the surface or sub-surface; exposed rock, earth, ice and other natural objects; cliffs; crevasses; encounters with wildlife and domestic animals; collision with trees, fences, equipment, vehicles, or other natural or artifical objects; collision with other persons or objects, including being struck from behind or being run over by skate blades; being ejected from tube; failure to keep limbs within a cart; failure to keep seatbelt buckled or other safety equipment worn properly; being struck by ball; inability to control one’s speed and direction; the possibility of fatigue, drowning, chemical reactions, burns, allergic reactions, straining, over-exposure to the cold, exhaustion from physical exertion; failure to follow instructions relating to the proper use of equipment; condition of and hidden defects in the equipment; failure or malfunction of equipment.
The risk of sustaining injuries result from the nature of the activity and can occur without any fault of either the student, or the school board, its’ employees/agents or Blue Mountain Resort where the activity is taking place. By choosing to take part in this activity, you are accepting the risk that you/your child may be injured. The chance of an injury occurring can be reduced by carefully following the instructions at all times while engaged in the activity. If you choose to allow your child to participate in the visit to Blue Mountain Resort, you must understand that you bear the responsibility for any injury that may occur.

RESPONSIBILITY CODE
There are elements of risk that common sense and personal awareness can help reduce. Please adhere to the code listed below and share with others the responsibility for a safe experience.
- Observe and obey all posted signs and instructions from staff.
- Do not stop on track until finish area - except in case of emergency.
- Seatbelt must be worn at all times. Removal of seatbelt may result in serious injury or death.
- You must not use coaster if your ability is impaired through use of alcohol or drugs.
- You must control your speed keeping a safe distance behind the cart ahead. Tailgating is prohibited. If track is wet or icy you must increase the distance between carts due to longer braking distance.
- You must not collide with the cart ahead of you.
- Keep hands on both brake handles and arms and legs inside cart at all times.
- Face direction of travel at all times. Never turn around, lie down, kneel or stand.
- If ride stops you must remain in cart with your seatbelt fastened until ride starts again or you are instructed otherwise by staff.
- If you are involved in or witness a collision or accident, you must remain at the unload area and identify yourself to attendant.

Canopy Climb Net Adventure has suspended rope nets, slides, tree houses and watchtower. Closed-toe and closed-back shoes are required, ski/snowboard boots not allowed. Running shoes are recommended. Be mindful of other guests, horseplay will not be tolerated. Only bounce/play on base netting, no climbing on side (vertical) netting. Guests aged 6-12 may access the attraction alone but must be supervised by an adult at ground level (Max 5 kids to 1 adult).
TUBE PARK RULES
- No tubing in the park without a valid tubing ticket
- Tubing Responsibility Code must be adhered to at all times while in the tube park
- Ski boots are not permitted to be worn while tubing
- Unsafe conduct will result in the removal of ticket without refund
- Only Blue Mountain Resorts’ tubes are allowed in the tube park
- Blue Mountain Resort is not responsible for damage or wear to clothing while tubing
- One person per tube only
- Tubing is only permitted in the designated tube park
- Listen to instructions from Blue Mountain staff at all times
- Walkup and tube down only in designated areas as instructed
TUBE PARK RESPONSIBILITY CODE
There are elements of risk that common sense and personal awareness can help reduce. Regardless of how you decide to use the slopes, always show courtesy to others, observe the code listed below and share with others the responsibility for a great outdoor experience.
- Only one person per tube in sitting position only.
- Before starting your descent, make sure the track is clear. Upon completion of your descents clear the track immediately, exiting at marked areas only. Do not cross other tube tracks.
- Do not stop where you obstruct a track or are not visible from above.
- If you are involved in or witness a collision you must remain at the scene and identify yourself to a patroller or other Blue Mountain staff.
- You must keep off closed tracks and obey Blue Mountain staff, signs and warnings at all times.
- After completion of tubing return the tube to the storage corral. Tubes must stay within the park boundaries at all times.
