Liability Waiver Form
In consideration of your acceptance of this form, I for myself, my heirs, executors, administrators and assigns, forever waive, release and discharge any and all rights, claims for damages, unknown, that I may have against 805 Defense, volunteers and other participants for any and all injuries in any manner arising or resulting from my participation in self defense classes. I attest and verify that I have full knowledge of the risks involved in any self-defense exercises and that I freely and voluntarily assume those risks as incident to participation, and that I will assume and pay my own medical and emergency expenses in the event of accident, illness, or other incapacity, regardless of whether I have authorized such expenses, and that I am medically and physically fit and able to participate in classes. I also give consent to 805 Defense, its owners, instructors, volunteers and other students to give minor emergency first aid and/or advice to me. I hereby release claims I have towards them for giving me such treatment.
I will make the instructor aware of any medical conditions or physical limitations before class. If I am pregnant, become pregnant, or am post-natal or post-surgical, my signature verifies that I have my physician's approval to participate. I also affirm that I alone am responsible to decide whether to participate, and any participation is at my own risk. I hereby agree to irrevocably release and waive any claims that I have now or may have hereafter concerning 805 Defense, its staff or instructors during this class.
I understand that all reasonable precautions are taken during class to provide a safe environment; however, I recognize that there are certain inherent risks associated with practicing self-defense techniques and I assume full responsibility for personal injury to myself. I further release and discharge 805 Defense for injury, loss or damage arising out of my use or presence upon the facilities used by 805 Defense whether caused by the fault of myself, my family, 805 Defense or other third parties.
I understand the 805 Defense program has been designed to provide me with the safest and most effective way to survive and escape a physical assault. However, I understand that the instructors cannot guarantee my safety through the use, or misuse, of the techniques taught in the class. I understand that I am responsible for the use of the knowledge and techniques and any possible injury or damages, which could result.
Media release: I give 805 Defense permission to use, reprint, and reproduce any audio, video, or other photographic recordings taken of participant during Training. These images may be used in promotional materials such as websites, Email, blogs, online newsletters, other social media, and other video, audio, or print advertising.
